that can be easily made,sorry your having problems traking one down! will need one piece to go by.

Couldnt you use a pipe repair plate to start making something close? There couple inches wide and conform to a pipe with a rubber liner to stop a leak.

much easier to start from scratch using same thickness of material, rather than try and alter something.
