Here is one I recreated for a customer from a supplied photo of an original S.O. decal and a Tax decal. On the pump in my reference photo, these were used together.
This was on a Chevron pump.

Jim, thanks for the reply, it is much appreciated. Below is a photo I have of two Chevron pumps with the decals. I know the bottom one was for taxes, I just haven't found one for Utah yet. Utahbob  Uploaded with ImageShack.us
