actually approx. 11 15/16" x 26" / has been cut down on one end / opposite end is original / has chips at bottom

JMO, but I don't think it was cut down. It looks more like a frost crack. I have four like that. Very common up north. Water gets along the rim and freezes. Most likely it was a 12 x 27 at one time before the piece broke off.
