Have had this for awhile, but do not know its name. Looked up the numbers on google and came up with nothing. Goes from 3/4" on the back side to 3/4" in the center on the front and (4) 1/2" threaded holes in the front. Thought it was maybe for some station light or something.

Are the four holes threaded?

Yes, all the holes are threaded Jack. Still trying to find out if you have that inner body also.

Craig , I think what you have is a lamp fixture adapter, junction box for four adjustable spot lamps. The two small holes are drain holes , should be facing down. The Mark with the circled W is I believe hall mark for Westinghouse.
