Ideas where I might find the plastic plugs for these holes on the top piece?

Any hardware store should have them..They have a slot in them,(not wrench plug), & are still used in the electrical industry today.
I bought a hand full for around 10 cents a piece 2 years ago @ ACE.

That is the exact original design they used back then.
